Former, Living American Military Vietnam POW

Former, living American military Vietnam POWs from the Vietnam War as listed by the Department of Defense:

1. Name, branch of Service, and retired rank (includes a medical retirement) of the former Vietnam POW to be placed on the certificate. If not in retired status, no rank will be placed on the certificate.
2. Event date to be placed on the certificate.
3. Name, address, and phone number to whom the certificate is to be mailed for presentation purposes if different than the requestor (phone numbers are needed because they are required by some mailing sources).

If an event date is not included in the request, the processing date will be placed on the certificate. When planning a presentation ceremony, please factor in the processing time for these certificates and allow up to 60 days prior to the event date for processing. The Commemoration processes requests for these certificates primarily through its commemorative partner organizations so that the certificates may be presented on behalf of a grateful nation.
